1. Application form
- The designated application form must be used. if you need the 2011 Application Form, please contact us through the bottom contact form

2. Official transcript:
　　　(An official or certified copy of your current transcript of study to date.)
- Copies of transcripts or unsealed transcripts will not be accepted

3. One letter of recommendation from a professional reference (i.e. university professor)
- Applicants are NOT permitted to inspect the letter.
Unsealed letters of recommendation will not be accepted from the applicants.
We are unable to accept unsigned letters of recommendation.
Hand-written letters will not be accepted.

4. One photocopy of your identification card
- Student IDs or the front page of your passport will be accepted

Please confirm Application procedures

20th April 2011 (JAPAN TIME)
The application and documents required must be submitted before that date. (Japan Time)

・Applications must be typed. Hand-written applications will not be accepted.
・Numbers: Please do not use roman numerals (i.e. I., II., III)
・JCI Member is not eligible for applying for any reason
・Do not abbreviate proper nouns. Write out the full name.
・Your name: Write your full name as it appears on your passport.
・Any applications after the deadline will not be accepted.
・An application with incomplete materials will be invalid.
・Applications will be used solely for the PCY program and will not be returned.

Acceptance/Rejection letters from JCI Osaka will be sent to the applicants via e-mail by the end of June 2011.

Participants' round-trip air tickets (economy-class section), transportation fees in Japan, accommodation facility and meals during the program will be provided by JCI Osaka.
